This shouldn't happen. A layout is just a Rainmeter.ini file in a subfolder from the C:\Users\[User]\AppData\Roaming\Rainmeter\Layouts folder, for a standard Rainmeter installation, so you can easily check if the file or positions you load are correct by opening such files with your favorite text viewer / editor. Maybe you have multiple monitors that swap between them because of different turn on times at startup?I changed the xy positions of some of my skins and saved it as a new layout.
When I tried to load the new layout or whenever the PC restarts, it load the old xy position. I will need to open Rainmeter and click "refresh all" for the new positions take effect.
